This talk aims to broaden the horizon of CKD care by advocating for a truly holistic, patient-centered approach.
Drawing on recent evidence and clinical best practices, we will explore how multidisciplinary teams can collaboratively support CKD patients throughout their journey. This course will look into innovative care models, early intervention and emerging digital tools that are reshaping how we deliver comprehensive CKD care. By shifting from a reactive to a proactive and personalized care paradigm, we can not only improve patient outcomes but also enhance quality of life and reduce healthcare disparities across diverse populations.
